Hospice Aide - PRN

Job Title: Hospice Aide
Location: San Antonio, TX

Position Overview:
We are seeking an experienced and compassionate Hospice Aide to join our team. The HA is an integral part of the hospice team. The care the HA provides is so important to patients and family members. The HA will be in clients' homes as well as facilities. The job duties would include.

Personal care: bathing, dressing, grooming and other hygiene needs.
Helping with tasks such as eating, mobility and toileting.
Light housekeeping to include making beds and cleaning.
Charting patient encounters
Reporting any concerns to patients RN Case Manager
Provide patients and families with resources and referrals to other community agencies

Qualification:

Certified Home Health Aide
A minimum of two years hospice experience
Excellent communication skills
Dedication to the hospice philosophy and values
